My personal choice to solve the anti-pattern #5 would be having most of the team pitching in the hiring process (as much possible). This not only reduces bias to a good extent but also helps promote autonomy/responsibility.
How can positive pattern #6 be implemented in reality, would something like a set of small OKRs help here or are there better ways.
