it's likely the webserver has no access to the profile image or the file is missing (broken image).
Right click on the unloaded image and click Open image in a new tab or View image... (something like that depending on your browser).
If the image doesn't open correctly then it's the link that's broken. Otherwise, you need to reupload the image or change permissions of the directory.
