I think learning to concentrate is a difficult thing, but doing meditation helps! Also, set yourself up an environment (the physical space you're in, but also your computer) with minimal distractions, this way you can more easily focus on the task at hand. If you are disciplined about these things, you'll start to train yourself and build a rhythm.
Start by incorporating a fitness routine. It doesn't matter what type of fitness routine but do your best to show up. By forcing yourself to show and exercise in that routine you start to develop the mental strength to work past issues.
If you think this is not enough and want a challenge. Go for a combat sport like Brazilian Jiu Jitsu for 2-3 times a week or go for trail running around your area once a week.
Be positive about getting feedback - it can be harsh, but your goal is to learn from it.
Getting your first job - you might to get hundreds or even thousands "No", but all you need is one "Yes". When it comes to large numbers, it will come eventually. Always do some retrospective with yourself or even ask the recruiter, what you should improve for your next interview.
Get prepared to the interview - interview is just another test, and this one you should be prepared to. Ask the recruiter what is the agenda so you can come prepared. Learn to answer code question, how to present your projects, and how to answer questions about personality.
When your self esteem on the lower side, remind yourself the wins.
