I don't think these points are terribly correlated with agile one way or the other. No version control is basically criminally negligent at this point. The processes are clearly mismanaged, the CTO sounds like someone technical who fell into the position. There is no excuse in having only a few clients. Software shops need to ensure quality, and if they don't you just get this bug churn. I'm guessing the codebase is also a huge ball of mud. Sadly all too typical.
